چکیده

Patients with schizophrenia are at higher risk for medical illnesses than people in the general population. The risk of atherosclerosis and cardiac death in these patients is markedly higher and parallels the severity of the disease. Last but not least, one should mention the role of the treating physician in the unwanted occurrence of abnormal QTc prolongation and SCD, mostly resulting from torsade de pointes (TdP). There is no question that the most effective way of preventing complications due to drug-induced QTc prolongation is to avoid the use of these drugs. However, there are many QTcprolonging drugs which have certain therapeutic advantages which the physician usually does not want to miss. In these cases, a systematic approach seems advisable. First of all, the physician prescribing such a drug must be familiar with the problem of drug-induced TdP. Moreover, the physician should also know that the risk for abnormal QTc prolongation and TdP varies between patients; although some patients may have a high propensity to the development of drug-associated TdP, the risk may be low in others. Risk factors for the occurrence of the arrhythmia have been identified (e.g. female gender, bradycardia, and hypokalaemia). Thus, although the role of regulatory authorities in pharmacology is crucial, we should not forget the role of the prescribing physician.

Apical Hypertrophic Cardiomyopathy in a Case with Chest Pain and Family History of Sudden Cardiac Death: A Case Report

Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiovascular disease, which is caused by a multitude of mutations in genes encoding proteins of the cardiac sarcomere (1). Apical hypertrophic cardiomyopathy (AHCM) is an uncommon type of HCM. The sudden cardiac death is less likely to occur in the patients inflicted with AHCM (2). Herein, we presented the case of a 29-year-old man ...

Histomorphological Spectrum of Various Cardiac Changes in Sudden Death: An Autopsy Study

Background and Objectives: In spite of large number of studies conducted so far, sudden cardiac death remains an enigma and relative importance of acute coronary events as a trigger of sudden death is currently unclear. An autopsy study of heart was therefore planned to observe various histomorphological cardiac changes, and to determine the frequency association of acute coronary events and my...

Performance of electrophysiologic study in an asymptomatic patient with type 2 intermittent Brugada syndrome: To do or not to do?

Background: Brugada syndrome (BrS) is an inherited channelopathy, which is associated with sudden cardiac death due to rapid polymorphic VT or VF. There is no definite consensus regarding the management of asymptomatic patients. Some experts advocate close follow-up; others propose the programmed stimulation for risk stratification. We aimed to evaluate the benefit of complete atrial and ventri...

Drug-induced long QT syndrome and fatal arrhythmias in the intensive care unit.

Long QT syndrome (LQTS) is a genetic or acquired condition characterised by a prolonged QT interval on the surface electrocardiogram (ECG) and is associated with a high risk of sudden cardiac death because of polymorph ventricular tachyarrhythmia called Torsade de Pointes arrhythmia. Drug-induced LQTS can occur as a side effect of commonly used cardiac and non-cardiac drugs in predisposed patie...
